About Us
Community Alliance Health Partners is dedicated to providing high quality healthcare services that are accessible and affordable for all members of our community, regardless of their socioeconomic status.
We serve people across the life-span – children, adolescents, adults, and seniors – with a particular focus on reaching those most vulnerable and under-resourced including individuals experiencing homelessness, those living with mental health and substance use challenges, and those with co-morbid mental health and physical health conditions.

We formally incorporated in 2022 as a separate, non-profit organization to further broaden access to physical health services for individuals and families across the community while sustaining an integrated, whole person approach.
Community Alliance Health Partners is governed by an independent Board of Directors, with majority membership comprised of patients of the primary care clinic. This helps assure ongoing responsiveness to community needs and continuous focus on sustained commitment to high quality medical care.
As our name suggests, we take a partnership approach in providing holistic care, working collaboratively with, among others, Community Alliance’s Center for Mental Health, Heart Ministry Center, Methodist Health System, Douglas County Health Department, and area emergency shelters to address your physical, mental, and social well-being. Most importantly, we partner with you – the individuals and families we serve – to address your current needs and help to achieve a healthier you.
Community Alliance Health Partners is recognized as a tax-exempt, charitable organization under Section 501(c)(3) of the Internal Revenue Code. All contributions are tax deductible to the extent permitted by law.
Primary Care Provider

John Brady, MD
Dr. Brady started at Community Alliance in 2022 after working in private practice and at a community health center in Fremont. As a family practitioner, he has extensive experience in working with individuals of all ages and dealing with a full range of medical needs. Among his many outside interests is joining friends in playing the bagpipes at area festivals and parades, a treat he’s brought to the office at times for the enjoyment of patients and staff alike.
