Patient Registration and Orientation
Welcome to Community Alliance Health Partners. This document outlines important steps and information to help you get started with your care and on your path to feeling better.
What You Can Expect as a New Patient
As a new patient, you will:
- Complete a medical history form so your primary care provider can provide the best possible care.
- Provide accurate emergency contact details, as well as any relevant contacts such as a legal guardian, representative payee, or individual with power of attorney.
- Present your insurance information and ID cards for scanning.
- Be fully registered before your first provider visit (except in emergency situations).
- Complete all necessary paperwork and signatures prior to your initial appointment, including:
- Notice of Privacy Practices
- Consent to Treat
- Authorization to Release Information forms
- Assignment of benefits
- Have your information securely entered into our electronic medical records system.
Your First Appointment
We strongly recommend a full physical exam during your first visit to provide your medical provider with a comprehensive understanding of your overall health.
Affordable Care Options
We are committed to making healthcare accessible. If you are concerned about costs, ask your Care Coordinator about our Sliding Fee Discount Program, which offers reduced fees based on household income and family size in accordance with federal poverty guidelines.
Additional Costs
We will inform you in advance of any out-of-pocket costs for supplies or equipment related to your care that are not included in the standard service (e.g., eye glasses, prescription medications, crutches).

- Monday, Tuesday, Wednesday, Friday: 8:30 AM – 5:00 PM
- Thursday: 8:30 AM – 7:00 PM
